you log onto the Sundry Product Catalog, Promotion Catalog, Price
List pages
and the Save Bucks Center on this website.
- In order to use
this form you must have a Maly's Midwest account number,
- You must acquire
a usercode/password from customer service to access these pages,
- You must have a
valid e-mail address so a copy of your order can be sent to you,
- The form allows
you to enter 124 items but you can e-mail multiple forms for 1
order,
- All orders are shipped
UPS via ground and take 1 to 3 business days for delivery,
- All orders are shipped
to the address that is registered with your account number,
- Orders will be billed
according to how your account is set up on our system,
- All orders have
shipping and handling charges,
- Use the <tab>
key for changing fields, the <return> key will actually
e-mail the form,
- If you ever have
