Establishing an account number with Maly's

In order for a client to have an order shipped out from our warehouse, they must establish an account with us. This
is true for all booth rental stylists as well as employees and salon owners. Each account must have an individuals
name on it. We will not open any accounts without an individuals name listed.


To set up an account you must:

Fax or mail a copy of your cosmetology license, a copy of your driver's licenses, and the salon/business license of
your place of employment. This information must be sent care of Sue Anderson. Once this information is received
and processed you will receive a Maly's ID card with your new account number on it, as well as a listing of our store
locations and contractual information in the mail.

* Please note all new accounts will be set up as C.O.D. accounts unless otherwise noted.

Payment Options

COD:
Maly's can ship your order to you COD. All COD orders include an additional $6 COD fee. All new accounts will be
set up as COD accounts unless otherwise noted.

Smart Pay Options

SMART CHECK:
Maly's will allow you to accept and pay for your order when delivered with only a signature. Let Maly's do the leg
work. If your checking account number is on file with us, your total order can be debited to your account no sooner
than a check would clear.

SMART CARD:
Maly's will accept all forms of credit card transactions including, Visa, MasterCard, American Express, and Discover.
You may also use your Visa check card. Many of these cards have rewards programs or rebates you can benefit from.

CHECK BY PHONE:
Maly's will accept your order when you provide your bank account and routing number (from your check). Funds will
be taken from account within 2 business days after your order is placed.

These methods offer many benefits such as:

-Less daily and weekly paperwork
-Orders may be left if you are not there
-No C.O.D fees
-No Interest Charges
-No Hassles!
